For whatever information you want to change, upload any one valid document (like Voter ID, Passport, Ration Card). Submit the form, you will get a URN number with which you can check your status. Note: For this, your mobile number must be linked to Aadhaar. Do not delay. Do it, otherwise… you may have to pay a heavy fine for PAN-Aadhaar linking after 31st December.
