The report of the parliamentary committee formed to investigate the death of the country’s first CDS General Bipin Rawat has come out. The report blames ‘human error’ for the Mi-17 V5 helicopter crash on December 8, 2021. Army helicopter crashed near Coonoor, Tamil Nadu. Everyone in the helicopter, including CDS Rawat and his wife, lost their lives.
Standing committee shared report
In a report tabled in Parliament on Tuesday, the Standing Committee on Defense shared data on the number of accidents involving Indian Air Force aircraft during the 13th Defense Plan period. The report says that a total of 34 accidents occurred from 2017 to 2022.
This includes nine aircraft accidents of the Indian Air Force in 2021-22 and 11 aircraft accidents in 2018-19. The report says that all these incidents occurred due to human error.

- General Rawat’s helicopter crashed on a hill near Coonoor, Tamil Nadu. After three years, the report of the Standing Committee on Defense Affairs has been presented in the Lok Sabha. In this, the cause of the incident has been stated to be human error (aircrew).
- The team investigating the incident had initially said that the accident occurred after clouds entered the valley due to unexpected changes in the weather. Due to which the pilot could not find the correct location and lost his way.
- The investigation team examined the flight data recorder and cockpit voice recorder to determine the probable cause of the accident. Apart from this, the team also interrogated all the available witnesses. After which complete information has been presented.

The country’s first CDS chief General Vipin Rawat was on his way to the Defense Services Staff College, Wellington from Sulur Air Force Base in Coimbatore, Tamil Nadu, along with his wife Madhulika and 12 other armed forces personnel on December 8, 2021. They were traveling in a Mi-17 V5 helicopter, when the helicopter crashed into the hills before landing. In this incident, all the people aboard the helicopter have died.
