Prayagraj. Doctors of the Urology Department of Swaroop Rani Nehru Hospital in Prayagraj, Uttar Pradesh, have given new hope to a 40-year-old patient in critical condition by performing a complex operation on his left kidney and removing 37 stones at once. This complex surgical procedure was performed at the PMSSY Urology Operation Theater of SRN Hospital on Friday. The patient had large stones in both his kidneys and his condition had become critical due to the infection. Giving this information on Saturday, Prof. of Motilal Nehru Medical College. Dr. Santosh Singh, Associate Professor of Swaruprani Nehru Hospital, said that Shatrughan, a resident of Panasi village of Rewa district of Madhya Pradesh, was suffering from the problem of stones in both the kidneys for about a year. According to family members, he was taking Ayurvedic and homeopathic medicines for the treatment of stones. About a month ago, he started having high fever and trouble breathing. When his condition worsened, he was brought to Swarup Rani Nehru Hospital, where after examination, doctors found him to be suffering from serious infection i.e. sepsis and kidney failure.
The patient was admitted to the ICU and put on ventilator. He also underwent hemodialysis while on ventilator support for three days. After some improvement in his condition, he was put on regular dialysis. During examination, large stones ranging from 4.5 to 6 cm were found in both the kidneys. The patient’s serum creatinine was 4.2 at the time of operation.
Considering the serious condition of the patient, the urology team decided to operate on the left kidney first. Anatrophic pyelolithotomy was performed on 21 August. During this procedure, the kidney was opened in a controlled manner and all 37 stones were taken out one by one. The kidney was then repaired and a 5/26 double-J (DJ) stent was inserted. During the operation, the renal artery was clamped in a controlled manner for about 20 minutes.

He required a ventilator and dialysis due to sepsis and kidney failure. The operation was challenging due to the presence of large stones in both the kidneys and creatinine being 4.2. After the patient’s condition improved, the left kidney was operated on first. Through anatrophic pyelolithotomy, 37 stones were removed and the kidney was repaired in a single operation. At present the patient is being medically monitored. Prof. was in the urology team that performed the operation. Dr. Shirish Mishra and Dr. Gajendra were included along with Dilip Chaurasia.
