We’ve long been told that physical activity is good for your brain. And while that’s certainly true, a major new study suggests that where and why you exercise may be just as essential as how much you do it.
A new meta-analysis published in The Lancet Public Health analyzed data from 74 studies and more than 4.2 million adults and found that leisure-time physical activity was associated with a 24% lower risk of dementia, while physical activity at work was linked to a 20% higher risk. Here’s what the researchers found and what it means for your brain health.
How Was the Study Conducted?
Researchers at the University of Southern California led a systematic review and meta-analysis of 74 cohort and case-control studies covering 4,227,297 participants across 30 countries, followed for a median of 10 years. Participants ranged in age from 45 to 93.
The research team looked to determine whether dementia risk differed depending on the context—or domain, as the researchers called them—in which physical activity occurred. The four domains examined were leisure-time activity (recreational exercise like walking, swimming or going to the gym), occupational activity (physical labor at work), household activity (such as cleaning or cooking) and active commuting (walking or cycling to work). Researchers tracked outcomes of all-cause dementia, Alzheimer’s disease and vascular dementia.
What Did the Study Find?
Overall, higher physical activity was associated with a 20% lower risk of all-cause dementia, a 21% lower risk of Alzheimer’s disease and a 29% lower risk of vascular dementia. But when results were broken down by domain, they found the following differences among domains:
- Leisure-time physical activity: 24% lower dementia risk
- Household physical activity: 15% lower risk—though this came from only one study
- Active commuting: 10% higher risk—based on two studies
- Occupational physical activity: 20% higher risk—based on five studies
The researchers found that not all movement is equal when it comes to lowering dementia risk. They dubbed this the “physical activity paradox,” a pattern already well established in cardiovascular research, where occupational activity consistently fails to deliver the same benefits as recreational exercise and can sometimes increase risk.
As to why this would be the case, researchers hypothesize that workplace physical activity tends to be repetitive, low-autonomy and performed without adequate recovery. Additionally, workers in physically demanding jobs may also be more likely to face lower socioeconomic status, workplace hazards, chronic stress and limited healthcare access, all of which can increase dementia risk.
Limitations
All of the included studies were observational, so these findings reflect associations rather than proof of causation. The occupational activity finding is challenging to separate from the socioeconomic factors that cluster around demanding physical work. The household and commuting findings were also founded on very few studies. Reverse causation is another concern, where people in the early, preclinical stages of dementia may naturally reduce activity before diagnosis, which could potentially distort the observed associations.
How Does This Apply to Real Life?
Regularly engaging in recreational physical activity that you choose to do and enjoy doing is the best type of exercise for your brain health. Examples of this include walks after dinner, gym classes, swimming, dancing or gardening for pleasure.
“When it comes to protecting the brain, the context of physical activity may matter just as much as movement itself,” lead study author David Raichlen, Ph.D.said in a statement. “Improving access to safe parks, walking trails and recreational facilities, ensuring people have adequate leisure time for exercise, and reducing harmful workplace exposures may ultimately prove as important as promoting physical activity itself.”
