Rising invasive fungal infections pose an emerging public health challenge
New Delhi: While bacterial and viral infections often dominate public health discussions, medical experts are drawing attention to another growing threat—invasive fungal infections. Once largely associated with superficial skin conditions, fungal diseases are increasingly causing severe infections affecting the lungs, bloodstream and brain, particularly among people with weakened immune systems.
Doctors warn that these infections are becoming more common, harder to diagnose and, in some cases, increasingly resistant to available antifungal medicines. In India, environmental conditions, a large population of vulnerable individuals and expanding urbanisation have combined to create favourable conditions for the spread of invasive fungal diseases.
A growing concern in healthcare
Invasive fungal infections occur when fungi enter the body and spread beyond the skin or mucous membranes to internal organs.
Unlike common fungal infections affecting the skin or nails, invasive infections can become life-threatening if they reach the bloodstream, lungs or central nervous system.
Medical experts note that one of the biggest challenges is delayed diagnosis. Early symptoms—including persistent fever, cough and breathlessness—often resemble bacterial or viral infections, making fungal diseases difficult to identify during the initial stages.
As a result, treatment may begin only after the infection has progressed significantly.
Drug-resistant fungi increasing concern
India is among the countries where Candida auris, a multidrug-resistant fungal pathogen, has emerged as a significant healthcare concern.
The fungus has been linked to outbreaks in healthcare facilities and is known for its ability to resist several commonly used antifungal medications, making treatment more challenging.
Another fungus, Aspergillus, which is commonly found in soil, dust and decaying organic matter, can cause severe lung infections, particularly among hospitalised patients and individuals with weakened immunity.
Doctors also warn about cryptococcal meningitis, a fungal infection affecting the brain and surrounding tissues, which can carry a high risk of death if diagnosis and treatment are delayed.
Environment contributing to increased exposure
Fungal spores are naturally present in the environment and are routinely inhaled by people without causing illness.
However, experts say several environmental factors may be increasing both exposure to fungal spores and susceptibility to infection.
Rapid urbanisation, continuous construction activity and increasing levels of air pollution contribute to higher concentrations of airborne dust containing fungal spores.
Construction sites can release large amounts of Aspergillus spores into the air, while monsoon flooding and poor drainage encourage mould growth in homes, hospitals and public buildings.
Climate change and rising temperatures are also believed to be altering the distribution of certain fungal species, allowing them to spread into regions where they were previously uncommon.
Pollution and respiratory health
Medical experts note that prolonged exposure to fine particulate air pollution may weaken the respiratory system’s natural defence mechanisms.
Healthy lungs are generally able to trap and remove inhaled fungal spores before they cause infection.
However, damaged airways and chronic respiratory conditions may reduce the body’s ability to clear these organisms, increasing the likelihood of fungal diseases in susceptible individuals.
High-risk groups require greater vigilance
Although healthy people are usually able to fight off fungal infections, certain groups face a substantially higher risk of developing invasive disease.
These include:
- Patients undergoing chemotherapy.
- Organ transplant recipients.
- Individuals receiving long-term steroid therapy.
- People admitted to intensive care units.
- Individuals with poorly controlled diabetes.
India has one of the world’s largest populations living with diabetes, making awareness of fungal infections particularly important among patients and healthcare providers.
Experts say many people remain unaware that diabetes and weakened immunity significantly increase the risk of severe fungal diseases.
Importance of early diagnosis
Doctors emphasise that recognising fungal infections early can significantly improve treatment outcomes.
Persistent fever that does not respond to antibiotics, unexplained respiratory symptoms or worsening illness in high-risk patients should prompt clinicians to consider fungal infections as a possible diagnosis.
Early laboratory testing and timely initiation of appropriate antifungal therapy can reduce complications and improve survival.
Healthcare professionals also stress the importance of maintaining a high level of clinical suspicion in vulnerable patients rather than assuming symptoms are caused solely by bacterial infections.
Need for greater awareness
Experts believe public awareness is essential to improving outcomes.
Patients with diabetes, cancer, organ transplants or other conditions affecting immunity should understand the symptoms of invasive fungal infections and seek prompt medical attention if symptoms persist.
Families caring for high-risk individuals are also encouraged to discuss concerns with healthcare providers and avoid delaying evaluation.
At the policy level, specialists say strengthening diagnostic capacity, improving hospital infection control and promoting awareness among both clinicians and the public will be important in addressing the growing burden of fungal diseases.
As invasive fungal infections continue to emerge as a significant healthcare challenge, experts stress that timely diagnosis, evidence-based treatment and increased public awareness will be critical in reducing illness and preventing avoidable deaths.
