Mumbai: Doctors at Wockhardt Hospitals Mumbai Central successfully performed a complex coronary angioplasty on a diabetic man in his 50s with massive calcium deposits in the arteries of the heart. Intravascular lithotripsy (IVL) is a state-of-the-art technique used for this treatment. Doctors were able to restore blood flow to the heart despite challenging conditions such as the complex structure of blood vessels and insufficient blood flow from the leg veins.

This patient was experiencing symptoms of coronary artery disease. An examination revealed massive calcium deposits in his coronary arteries. Further examination revealed that the blood flow needed to carry the catheter through the leg veins was also insufficient. Therefore, conventional treatment from leg veins would be challenging.

Increasing age and chronic diabetes increase the risk of calcium deposits in the coronary arteries. When calcium deposits build up in large amounts, the arteries become extremely hard. This can make conventional angioplasty technically difficult and increase the risk of the stent not being fully inflated. Parin Sangoi said, “Heavily calcified coronary arteries are one of the biggest challenges in interventional cardiology. Calcium causes the arteries to stiffen, and it is not always possible to properly prepare the artery before stenting with conventional angioplasty.”

Doctors successfully performed the procedure through the radial artery in the wrist, despite the complex structure of the blood vessels and insufficient blood flow from the leg veins. An intravascular lithotripsy (IVL) technique was used to reduce calcium deposits before stent placement. This helps in dilating the blood vessel more effectively.

Dr. Sangoi explained, “The IVL technique uses controlled sound wave pressure to break up the calcium deposits in the vessel wall into microforms. This makes the vessel more flexible and helps the stent expand better. If the right patients are selected, this technique can be an important option in cases where there is a large amount of calcium deposits and conventional angioplasty is difficult.”

The procedure was successfully completed and blood flow was excellently restored in the affected coronary artery. After treatment the patient was doing well and was discharged from the hospital in a stable condition.Dr. Sangoi added, “Technological advances in interventional cardiology are making it possible to treat increasingly complex coronary artery occlusions. Therefore, there is no need to resort to high-risk surgery every time.”
